In an effort to combat homelessness, UCAP created the Progressive Path Program (PATH), which offers permanent housing to homeless persons with disabilities and their families.. PATH provides comprehensive and supportive programs designed to enable individuals and/or families to:.. Obtain permanent housing.. Increase work skills.. Increase self-sufficiency, through educational programs, workshops, training and counselling services.. Rental units are provided in the community at a fixed location.. Case management is offered weekly through home visits.. Residents are required to pay 30% of their disability income.. Eligibility Criteria.. Prince George s County, Maryland resident.. Unaccompanied homeless individual (or resident of a homeless shelter) with a disabling condition who has either been continuously homeless for a year or more, or had at least four (4) episodes of homelessness in the past three years.. A family or a single individual with a disability,  ...   training that would increase their ability to earn a higher income.. The Progressive Path Program provides 22 apartment units for the chronically homeless and disabled individuals and provides stabilization for the disabled.. Transitional housing opportunities are also provided for the chronically homeless.. PATH case managers visit with clients at their apartments to provide supervision, support and therapeutic counseling..
