    Descriptive info: Serving Jackson,.. What We Do.. The Aging & Disability Resource Center of Western Wisconsin.. offers the following services:.. Information and assistance.. We provide information to the general public about services, resources, and programs in areas such as: disability and long-term care related services and living arrangements, health and behavioral health, adult protective services, employment and training for people with disabilities, transportation, home maintenance, nutrition, IRIS and Family Care.. Aging and Disability Resource Center staff will provide help to connect people with those services and to also apply for SSI, FoodShare, and Medicaid as needed.. Long-Term Care Options Counseling.. We offer consultation and advice about options available to meet an individual’s long term care needs.. This consultation includes discussion of the factors to consider when making long-term care decisions.. The Aging and Disability Resource Center offers pre-admission consultation to all individuals with long-term care needs entering nursing facilities, community-based residential facilities, and residential care apartment complexes to provide objective information about the cost-effective options available to them.. This service is also available to other people with long-term care needs who request it.. Benefits Counseling.. The Aging and Disability Resource Center provides accurate and current information on private and government  ...   Early Intervention.. The Aging and Disability Resource Center promotes effective prevention efforts to keep people healthy and independent.. In collaboration with public and private health and social service partners in the community, the Aging and Disability Resource Center offers both information and intervention activities that focus on reducing the risk of disabilities.. This may include a program to review medications or nutrition, a home safety review to prevent falls, or finding appropriate fitness programs for older people or people with disabilities.. Access to the Family Care Benefit and IRIS.. For people who request it, the Aging and Disability Resource Center will administer the Long-Term Care Functional Screen to assess the individual’s level of need for services and eligibility for long term care benefits.. Once the individual’s level of need is determined, the Aging and Disability Resource Center will provide information about the options available to him or her—to enroll a long term care benefit program (if eligible), to stay in the Medicaid fee-for service system (if eligible), or to privately pay for these services.. If the individual chooses Family Care or IRIS, the Aging and Disability Resource Center will assist with enrolling the person in the chosen program..

As a volunteer guardian you would be appointed to help a person who is unable to make independent decisions about their own general welfare.. These decisions could be about health care, finances, living arrangements, or social and supportive services.. As a volunteer guardian, you would be matched with someone whose needs fit your skills, abilities and interests..  ...   Center.. People who need help from a guardian may have a degenerative brain disorder, such as Alzheimer s disease, severe and persistent mental illness, developmental disabilities, or a similar problem.. Your assistance will help to ensure a good quality of life for someone who needs an advocate and has limited support from family or friends.. Volunteer guardians often find they enjoy long-lasting friendship beyond the duties of guardianship..     Descriptive info: La Crosse, Monroe,.. A brief history.. In 1998 the state of Wisconsin created a new and flexible way to deliver publicly funded long term care services called.. This system emphasizes independence and quality of life while recognizing the need for support.. Family Care serves elderly people and other adults with physical or developmental disabilities.. In each participating county, Aging and Disability Resource Centers provide information and assistance to help people connect with all kinds of services, benefits and community resources for elderly people, people with disabilities, and their families.. Aging and Disability Resource Centers also screen for eligibility for the Family Care and IRIS benefits.. Eligibility for each is based upon the degree to which a person’s physical or cognitive condition limits his or her ability to  ...   programs.. In addition to long term care options counseling for individuals with issues related to aging and physical or developmental disabilities, the Aging and Disability Resource Center also provides information and assistance to people having mental health and substance abuse issues.. The addition of these groups to the services of the Aging and Disability Resource Center began in 2004 as part of the Mental Health Redesign Project.. After many months of planning with the West Central Consortium group, the Aging and Disability Resource Centers in Jackson and La Crosse counties merged in July 2008 to form the Aging and Disability Resource Center of Western Wisconsin.. An office is planned to open in September 2008 in Viroqua (Vernon County) and another in November 2008 in Sparta (Monroe County)..
